| Service Description | The patient passport seeks to improve the patient journey for people with a learning disabilty through the acute hospital setting. It is not expected to hold all the information about the person, but is a snap shot of the person at the time, seeking to make a connection with people who know the person least (the acute staff) with those who know them best ie parent/ carer/ keyworker/ community nurse. The passport is available on a CD ROM with resources and websites that we take for granted within LD ie the reports from the national Patient Safety Agency; the Mental Capacity Act; Best Interests Proforma; consent pathway... etc |
